Back testing result errors

Hello Definedge Team and Respected Mentors,
I hope you all are doing well, I was back testing some strategies and found some discrepancies in results, so I back tested some individual stocks ( and checked them manually) and found that system is not working as per the conditions, For your reference I am attaching the result of
"Coforge", Entry condition - Golden Crossover ( SMA 50 above SMA 200) and Exit - Below supertrend
as per back testing entries (I am giving example of only 3-4, there are so many wrong entries , not only in Coforge but in other instruments as well) -
Golden Crossover was on
- 10 Aug 26 (right entry)
- 09 Feb 26 ( system took the entry , but it was SMA 50 below SMA 200, its wrong as per condition)
-05 Feb 26 ( system took the entry , for no reason)
-03 Feb 26 ( system took the entry, for no reason) and it goes on...Pls look into this and advise if I am doing it wrong or something else.

Respected Sir and Team,
I would like to bring your attention to this post. As I have mentioned many times that I appreciate a lot all the products of Definedge and the efforts the team has put to make it a wonderful platform but at the same time, I am very disappointed with this Forum, its not the first time but every time, one has to wait a long, and still need to tag the team and Brijesh sir to get even a single update and reply from the team. I used to be an active user, but seems , no one is interested with the feedbacks and query on this Forum. Hope you will think about it and try to improve.
